The Illinois Department of Public Health requires a number of immunizations and exams before students can attend school in the State of Illinois. Below is a detailed list of those requirements and the grades affected by each prerequisite. Parents are asked to please plan for the 2025/2026 school year and begin scheduling these immunizations and exams soon. If missing any required immunizations or a physical, students may be subject to exclusion from school starting October 15.
All students entering Preschool, Kindergarten, Sixth, and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Health form. A current physical is one that was completed within one year of August 11, 2025. Any student transferring from another state or country must have an Illinois physical, regardless of grade level. A sports physical does not meet this requirement. The parent portion at the top of the physical form must be completed and signed.
All students must have medical verification of a complete immunization history according to Illinois immunization requirements, including the following:
All students entering kindergarten or first entry of school being 1st grade must show proof of receiving four or more doses of DTP/DTaP, with the last dose being a booster received on or after the 4th birthday.
All students in Preschool must demonstrate proof of receiving three doses of DTP or DTaP.
All students entering kindergarten, first, second, third, fourth, and grades six through twelve must show proof of receiving two doses of Varicella (chickenpox) vaccine and two doses of MMR (Measles, Mumps, Rubella).
All students entering Preschool and Kindergarten must have proof of three or more Polio doses, with the fourth dose on or after the 4th birthday.
All students entering sixth, seventh, eighth, and ninth grade must show proof of receiving one dose of the Meningococcal (meningitis) vaccine.
Students entering sixth through 12th grade are required to have three doses of the hepatitis B vaccine, given at the appropriate intervals. However, the third dose is not required if documentation shows the student received two doses of the adult formulation Recombivax-HB vaccine (10 mcg) between the ages of 11 and 15, and at least four months have passed between the two doses.
All students entering twelfth grade must show proof of receiving two doses of Meningococcal (meningitis) vaccine unless the first dose was given after the age of 16, then only one dose is required.
All students entering grades six through eleven must show proof of receiving one dose of Tdap vaccine.
Dental Examination Requirement: Illinois Dental examination forms are due by May 15 of the year the student is enrolled in kindergarten, second, sixth, and ninth grade. A current examination is one that was completed within 18 months of May 15.
Eye Examination Requirement: Illinois Eye examination reports are due by October 15 for students enrolling in kindergarten or for the first time in an Illinois public school. A current examination is within a year of enrollment.
